PARTICIPATION AGREEMENT & WAIVER

By registering yourself or your child for any class, rehearsal, performance, competition, camp, clinic, workshop, private lesson, or other activity offered by High Voltage Dance Company ("HVDC"), you acknowledge that you have read, understand, and agree to the terms outlined in this Participation Agreement & Waiver.

1. ASSUMPTION OF RISK
⋅ Participation in dance, acrobatics, tumbling, conditioning, stretching, performances, competitions, camps, workshops, and related activities involves inherent risks of injury.
⋅ These risks may include, but are not limited to, falls, collisions, sprains, strains, fractures, concussions, and other minor or serious injuries.
⋅ By participating, you voluntarily assume all risks associated with participation in HVDC activities.

2. RELEASE OF LIABILITY
⋅ To the fullest extent permitted by law, you release and hold harmless High Voltage Dance Company, LLC, its owners, instructors, employees, independent contractors, volunteers, assistants, and representatives from any claims, demands, liabilities, damages, losses, costs, or expenses arising out of participation in any HVDC activity, except where prohibited by law.
⋅ This release applies to all classes, rehearsals, performances, competitions, camps, workshops, private lessons, studio events, and related activities.

3. MEDICAL INFORMATION
⋅ You certify that the participant is physically able to safely participate in dance activities.
⋅ You agree to notify HVDC of any medical conditions, allergies, injuries, restrictions, or other health concerns that may affect participation.
⋅ It is your responsibility to keep all medical and emergency contact information current.

4. EMERGENCY MEDICAL AUTHORIZATION
⋅ In the event of an accident or medical emergency, you authorize HVDC staff to seek emergency medical treatment if a parent or emergency contact cannot be reached immediately.
⋅ You understand that all medical expenses are the responsibility of the participant or parent/guardian.

5. PHOTO & VIDEO RELEASE
⋅ HVDC may photograph or record classes, rehearsals, performances, competitions, camps, workshops, and other studio activities.
⋅ Photographs and videos may be used for promotional purposes including social media, advertisements, printed materials, studio displays, newsletters, and the HVDC website.
⋅ If you prefer that you or your child not appear in promotional materials, you must notify the studio in writing. While HVDC will make reasonable efforts to honor your request, we cannot guarantee exclusion from group photographs or videos taken during public events.

6. RECORDING POLICY
⋅ Recording choreography, classes, rehearsals, or instruction without permission is prohibited.
⋅ Competition routines, choreography, music edits, instructional materials, and rehearsal footage may not be shared publicly unless authorized by HVDC.

7. CHOREOGRAPHY & INTELLECTUAL PROPERTY
⋅ All choreography, music edits, class curriculum, logos, graphics, written materials, videos, photographs, and other original content created by High Voltage Dance Company remain the intellectual property of HVDC and/or its choreographers.
⋅ Studio choreography may not be taught, reproduced, entered into competitions, performed elsewhere, sold, or distributed without prior written permission.

8. BRAND USE
⋅ The High Voltage Dance Company name, logos, branding, graphics, and trademarks may not be used without prior written authorization.
⋅ Unauthorized merchandise, promotional materials, fundraising items, or online use of HVDC branding is prohibited.

9. PERSONAL PROPERTY
⋅ HVDC is not responsible for lost, stolen, or damaged personal belongings, including dancewear, costumes, shoes, phones, jewelry, or other personal property.

10. WEBSITE & SOCIAL MEDIA
⋅ HVDC may maintain websites, social media pages, and other online platforms.
⋅ Comments, photos, or content posted by users on third-party platforms are not controlled by HVDC.
⋅ Families are expected to interact respectfully on all HVDC-managed online platforms.

11. FORCE MAJEURE
⋅ HVDC shall not be responsible for delays, cancellations, schedule changes, or interruptions caused by circumstances beyond its reasonable control, including severe weather, power outages, public health emergencies, government actions, natural disasters, or other unforeseen events.

12. GOVERNING LAW
⋅ This Participation Agreement & Waiver shall be governed by and interpreted in accordance with the laws of the State of Maryland.

13. ACKNOWLEDGMENT
⋅ By completing registration, you acknowledge that you have read and understand this Participation Agreement & Waiver.
⋅ You voluntarily accept these terms on behalf of yourself and any participating minor.
⋅ You understand that participation in High Voltage Dance Company activities is conditioned upon acceptance of this Agreement.
